One of our client, an Health Center located in Delaware, is looking for an Health Promotion Advocate.Position Type: ContractLocation: DelawareSchedule: MonFri 08:00am-04:00pm/09:00am-05:00pmEssential Responsibilities: The following duties are not intended to serve as a comprehensive list of all duties performed by all associates in this classification. Listed are duties intended to provide a representative summary of the major duties and responsibilities. Incumbent(s) may not be required to perform all duties listed and may be required to perform additional, position-specific duties. Develops or utilizes existing mechanisms to identify uninsured and underinsured individual responsibilities are present to Client. Utilizes resources available within the organization in order to maximize the number of uninsured and underinsured that are successfully referred and enrolled in social service and medical assistance programs. Contacts, screens, and counsel's patients and/or their family regarding financial eligibility for HCC, SFL, and/or other programs that provide free or reduced cost medical services. Promote disease prevention to include: Identify self-pay users of health care services. Serve as an advocate for patients and providers by determining and meeting their needs as well as educating them on the importance of screening and being active participants in their own health care. Implements activities that are educational and serves to encourage individuals to receive appropriate health screening. Identifies primary audience that are at higher risk, and are likely to be knowledge about screening guidelines. Aims to provide educational information and materials to these individuals. Develops and implements new recruitment strategies for underserved communities as necessary. Provides outreach assistance to inform local physicians of available healthcare services. Educates potentially eligible and willing participants about various programs application processes. Contacts uninsured and underinsured individuals to inform them of potential eligibility for HCC, SFL, and other medical assistance programs. Conducts initial screening to determine potential eligibility for HCC, SFL, and other medical assistance programs. Counsels patients on the application process and benefits of enrolling in HCC, SFL, and other medical assistance programs. Initiates appointments for individuals interested in scheduling a screening. Places both reminders and follow-up phone calls to these individuals to ensure that they receive the desired screening. Compiles and reports the data from activities in the required format. Ensures HIPAA enforcement and maintenance of confidentiality. Keeps appropriate program records, such as proof of income. Promotes health promotion and disease prevention programs and services. Acts proactively to identify and address problems as they arise. Performs other duties as assigned.Education and/or Experience: Associate's degree or equivalent combination of education and experience required. Minimum of one year of healthcare experience. Community resources and publicly funded community and health service program experience. Case management experience including tracking, follow-up, and evaluation. Health and Human services support to include interviewing patients and assessing personal health, social, and financial needs. Record keeping which includes maintain records, logs, and filing systems. Use of automated information system to enter, update, modify, delete, retrieve/inquire, and report data.
