Work Shifts ROTATING Job Description PURPOSE OF POSITION To Provide Pharmaceutical Services. AREAS OF RESPONSIBILITIES Pharmacy Department SUMMARY OF DUTIES AND RESPONSIBILITIES I. FUNCTION: Ensures patient safety by accurately and appropriately dispensing and preparing medications in the various pharmacy divisions; unit dose areas, outpatient dispensing and I.V. Admixture/Hyperalimentation. II. POSITION SPECIFIC COMPETENCIES: 1. Interacts with other staff members, patients and visitors, serves on interdisciplinary teams, provides clinical consulting and provides education to patients and their significant others. 2. Demonstrates skills needed to fill inpatient medication orders. 3. Demonstrates skills needed to dispense medications to outpatients. 4. Exhibits knowledge and skill needed to prepare sterile solution. 5. Exhibits knowledge and skill needed to prepare parenteral chemotherapy for delivery to inpatient and outpatient areas. III. MAJOR DUTIES: 1. Works closely with the interdisciplinary team in in-patient and Amb care and does clinical intervention, patient counseling, therapeutic recommendation 2. Helps develop protocols, conduct DUEs etc 3. Responsible for filling inpatient and Ambulatory Care drug orders. 4. Responsible for the completion of inspections as assigned by the Assistant Director of Pharmacy. 5. Participates in quality assurance programs. 6. Responsible for recording and monitoring complex I.V. Admixture/Hyperalimentation orders and transferring information to patient profiles. 7. Prepares I.V. Admixtures and Hyperalimentation orders using aseptic technique. 8. Periodically checks medications for valid expiration dates. 9. Participates in continuing education, in-service education, and Clinical Programs. 10. Rotates through the Unit Dose Pharmacy Inpatients areas, and participates in the following: a. Fills medication orders for unit dose carts in patient areas. b. Maintains complete unit dose patient profiles; checks for drug interactions, appropriate dosing of medication, allergies, and diagnosis, patient's weight, drug-food interactions, IV orders, and stat orders. c. Maintains adequate supply of unit dose medication. d. Prepackages unit dose medication not commercially available and maintains a prepackaging log. e. Prepares statistical data on a monthly basis (regarding workload and patient census). f. Responsible for the inspections in unit dose areas of the hospital. g. Answers telephone requests from physicians or professional staff for drug information and resolves problems related to established policies and procedures. h. Participates in therapeutic category surveillance studies and verifies appropriate prescribing practices & policies of restricted and non-formulary medications. i. Assist in other projects/duties as assigned by the Director of Pharmacy/designee 11. Rotates through the Outpatient Pharmacy and participates in the following: a. Screens and properly fills and dispenses prescriptions for outpatients. b. Answers telephone requests from physicians or professional staff for drug information and resolves problems. c. Orders appropriate quantities of medication through daily orders from the main pharmacy. d. Assists in narcotic inventory.
