Job DescriptionThe HSP LVL III, Inpatient Pharmacy Supervisor is responsible for the ensuring efficient workflow in the Inpatient Pharmacy Satellite.1. Supervises and/or coordinates the operation of a Pharmacy unit or major function.2. Prepares departmental reports, including the performance of statistical analyses.3. Develops and implements new processes to improve the operation of the Pharmacy and the qualityof care provided.4. Orients new staff members.5. Monitors and evaluates staff performance and follows appropriate disciplinary proceedings, asnecessary.6. Prepares unit work schedule.7. Ensures staff compliance with regulatory agency and health care setting rules and regulations.8. Responsible for the maintenance of adequate quantities of medication and other supplies in thePharmacy unit.9. Evaluates new drugs released by the FDA in terms of efficacy, adverse effect profile, and cost;makes recommendations on additions and deletions to the health care setting's drug formulary.10. Manages the medication therapy of a group or unit of patients as part of an interdisciplinary team,and participates in rounds.11. Coordinates the assessment of medication use.12. Serves as a professional and technical resource for medical professionals in specialty area(s)General tasks and responsibilities will include: 1.Prepares and dispenses medication orders per physician request according to established policies, procedures and protocols.2.Issues controlled substances to patient care areas and maintains records as required by law.3.Ensures safe, appropriate, cost-effective therapies for patients according to established policies, procedures and protocols4.Monitors medication therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug, dose and therapeutic duplications.5.Detects and reports suspected .adverse drug reactions and medication errors accurately and in a timely manner.6.Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitutions and promoting rational medication therapy selection.7.Provides accurate, adequate and timely drug information to the professional staff.8.Provides drug information to families and staff.9.Documents all clinical activities and interventions accurately and completely.10.Participates in the quality improvement and medication use review activities of the department; collects data; conducts quality monitors and inspections; and maintains log, records and other documentation as required.11.Contributes to the quality and effective operations of the department by: a.Supervising pharmacy personnel and verifying daily activities of pharmacy staff. b.Participating in the performance appraisal of pharmacy staff. c.Prepares reports on pharmaceutical care and outcomes.12.Responsible for reviewing Monthly Unit inspections.13.Provide Pharmacist coverage as requested by the Director of his/her designee14.Provides pharmaceutical education to professional staff at lecture meetings or individually. 15.Supervises and coordinates the Coordinates the activities of other supervisors ( HSP LVL II) in integrating the functions and staff activity in the pharmacy.16.Performs related duties as assigned.
Minimum QualificationsFor Level I: 1. Possession of a valid NY State license to practice pharmacy. For Level II: In addition to meeting the qualification requirements of the lower level: 1. Two years of post licensure experience; or 2. Completion of a post graduate specialty residency or fellowship program; or 3. Current Pharmacy Board Certification. For Level III: In addition to meeting the qualification requirements of the lower levels, an additional: 1.
