Job DescriptionA. Direct Patient Care•Participate in patient care rounds in assigned Surgical ICU•Comprehensive acute management of adult patients in the Surgical ICU•Review of medication profiles to assure optimal pharmaceutical care for all co-morbid conditions•Optimizing medication therapies to prevent and reduce medication errors•Therapeutic drug monitoring for appropriateness of use, dose, dosage form, and drug interactions•Provide pharmacokinetic consultations when requested•Provide patients with discharge counseling as applicable•Provide medication information to interdisciplinary Surgical ICU team•Develop medication guidelines, order sets, and protocols in collaboration with clinical staff•Pursue opportunities for optimization of Electronic Medical Record related to care of ICU patients•Document pharmacotherapy interventions within the Electronic Medical RecordB. Education responsibilities:•Provide patient specific education to patients as requested by medical staff or nursing personnel•Facilitate, develop, and implement lectures to pharmacy staff in critical care related topics•Provide in-services to medical staff on pharmacological topics•Provide in-services to nursing staff on pertinent pharmacy-nursing related topics•Development of guidelines and protocols related to medicine and critical care.C. Departmental responsibilities•Ensure safe, appropriate, cost effective medication therapies for patients according to established policies, procedures, and protocols•Facilitate the development and implementation of treatment guidelines, protocols, formulary changes as approved by appropriate committees. Ensure guidelines & protocols match national2standards and include clinically appropriate treatment standards, while respecting the cost structure of the institution•Coordinate the selection and safe use of alternative medication in response to medication shortages•Provide clinical consultation and clarification to practitioners•Support the pharmacy department's clinical endeavors•Review the use of targeted medication for compliance with pharmacoeconomic programs•Suggest appropriate, cost effective therapeutic alternatives to medical staff•Monitor drug therapy regimens for allergies, contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, adverse drug reactions, appropriateness of drug and dose, and therapeutic duplication•Help reduce or limit cost of medications utilized in the treatment of critical care patients through review of each specific patient medication regimen•Monitor cost of medication therapy for ICU patients and report to Section Chief and Pharmacy Leadership•Provide accurate, adequate, and timely drug information to professional staff•Contribute to the publishing of pharmacy newsletter•Supervise and conduct continued education to the pharmacists and pharmacy students•Prepare and review drug use evaluation criteria, adverse drug reaction reports, and pharmacy interventions summary to various committees•Participate in hospital education programs•Coordinate policies and procedures for clinical activities (i.e. therapeutic interchange, unacceptable abbreviations, look alike-sound alike drugs, medication events reporting etc.)•Participate in quality improvement activities of the department•Collect, analyze, and maintain data for all documented interventions•Establish productive, collaborative relationships among the multidisciplinary healthcare team•Sustain professional competence and promote development through participation in in-services, workshops, seminars, and professional organizations•Precept and mentor pharmacy students for local Colleges of Pharmacy•Train pharmacists and pharmacy students to participate in drug use evaluation•Facilitate discharge/transfer planning and transitions of care related to medication use•Complete special projects as assigned by Pharmacy Leadership•Participate in Pharmacy and Therapeutics committee through facilitation of drug formulary additions
Minimum QualificationsPharmD Pharmacy degree required•Active New York State pharmacy license and registration.•Active ACLS certification.•PGY1 Pharmacy Residency completion or previous hospital experience required•Critical Care PGY2 Pharmacy Residency or a minimum of 3 years of comparable practice experience required•Board Certification in Critical Care Pharmacy preferredNYC Health and Hospitals offers a competitive benefits package that includes:
