Job Description:
Overview:

To provide Basic Population Health Management services for members which includes engagingwith members individually or as part of a group to support lifestyle modifications to managechronic diseases, to identify care gaps and needed services, and to aid Partnership members inmaintaining wellness. Healthy Living Coaches shall also serve as a resource about Partnershipmember benefits as well as Population Health programs and services.

Responsibilities:
  • Proactively engage and connect with Partnership members through multiple modalities thatmay include phone, web-based tools, group presentations, and/or at community events, perPHM leadership direction.
  • Maintain records documenting outreach activities.
  • Perform Basic Population Health Management activities for assigned member caseload tohelp members identify opportunities to improve and support healthy lifestyle change usingmotivational interviewing strategies.
  • Refer members to appropriate screening tools, identify self-management tools, educationalmaterial, and special training needs. Monitor and track member use of referred resourcesand provide recommendations for resources needed. Document closed-loop referrals tocommunity resources, providers, and other available services.
  • Provide current information on Partnerships benefits, services, updates, and wellnessprograms, as appropriate.
  • Attend member-facing events to promote Partnerships presence in the community whilerepresenting Partnership in a professional manner per Partnership policies. Gather memberfeedback, and identify other agencies that provide resources to the members we serve.
  • Coordinate logistics of tabling assigned events including swag, incentives, member-facingmaterials, Partnership brand promotion (i.e. tablecloths, banners, etc.), and returningmaterials to Partnership inventory upon event completion.
  • Gather information and provide feedback per departmental guidelines to leadershipregarding the barriers members face accessing wellness care. Provide recommendationson means to promote members knowledge and resources to health and wellness.
  • Work with Health Education to identify appropriate member educational materials toshare with community partners, providers, and members.
  • Other duties as assigned.
Qualifications:

Education and Experience

Associates degree or one of the following certifications Required:Registered Pharmacy Tech (RPhT) or Medical Assistant or equivalentcertification. Prefer a minimum of three years in a Managed Medi-Calplan, current knowledge of Medi-Cal member benefits, or pass abenefits test within 90 days of hire.

Special Skills, Licenses and Certifications

Knowledge of business software applications. Preferred training inMotivational Interviewing or Lifestyle Coaching. Bi-lingual preferredin Partnership threshold languages (Spanish, Tagalog, or Russian).Valid California drivers license and proof of current automobileinsurance compliant with Partnership policy are required to operate avehicle and travel for company business.

Performance Based Competencies

Excellent written and oral communication skills. Must be able to workindependently. Ability to work with ethnically and culturally diversepopulations and contribute to a collaborative team environment.Requires patience and ability to work with people of all levels ofunderstanding and education. Requires excellent organizational skillswith the ability to prioritize assignments, maintain accurate records, and meet deadlines.

Work Environment And Physical Demands

Ability to use a computer keyboard. When required, ability to lift,carry, or move objects weighing up to 40 lbs. Local travel requiredseveral days per week; some weekend work and some overnight travelrequired (up to 20%).

HIRING RANGE:

$72,364.92 - $90,456.15
