Job Description:As a Heart Failure Care Coordinator at Prisma Health, you will be responsible for coordinating care across the continuum for patients with heart failure, collaborating with cardiologists, advanced practice nurses, and other healthcare professionals to ensure seamless care transitions and optimal patient outcomes. This role requires a strong background in cardiac nursing, with a minimum of two years of experience in this field. A graduate degree in Nursing is preferred, but a bachelor's degree is required. In addition to their educational qualifications, the ideal candidate will hold RN licensure in South Carolina and be certified in ACLS and BLS. Key Responsibilities:Key responsibilities of this role include developing a therapeutic relationship with patients and their families, facilitating access and utilization of community resources, teaching and developing curriculum to provide education reinforcement for heart failure patients and their families, overseeing data quality and performance improvement in the clinic setting, collaborating with the inpatient heart failure team to ensure care transitions, and monitoring and coordinating telemedicine data between clinic providers and patients. Requirements:To be successful in this role, you will need to possess a strong background in cardiac nursing, with a minimum of two years of experience in this field.
