Description I) Job PurposeProvides access and referral service coordination to families that call in to MHMR with children that are 6-17 years old. Ensures that all calls received are served timely and appropriately and in keeping with families' rights and needs. Connects callers to most appropriate internal and/or external resources. Works with families until ensured that all needs are being met or follow up is scheduled.II) Essential FunctionsA) Receives navigation referral calls from MHMR icare staff. Demonstrates excellent customer service as outline below.B) Researches and networks with referral sources in communities served by MHMR of Tarrant. Accesses ECS, MHMR, community, state and national resource data bases and online live binders for referral sources.C) Completes assessments with families to ensure referrals are appropriately made both internally and externally.D) Maintains list of local programs and resources that have capacity to serve families. E) Assists with updating of databases to include new service programs in the community for families.F) Follows up with families as needed to determine access and service provision.G) Documents calls and content of calls as required.H) Works within the Avatar EHR to admit families into the system.I) Schedules intake appointments for families that are referred for internal services through the Avatar calendar.J) Meets weekly with Navigation team and supervisor.K) Attends outside networking and referral source meetings as requested by supervisor.L) Works closely with CFS Quality Management Resource Specialist to share and network referral sources.M) Performance standards are performed as applicable with MHMR's We CARE values We Connect People in Our Community. N) Performs other job duties or responsibilities as requested or assigned.III) Knowledge of Laws, Regulations, Policies/Procedures, Skills, and AbilitiesA) Ability to recognize and respect cultural diversity of patients.B) Serve as the communication liaison between patient and prescriber, Capable of documenting patient communication and clinical treatment accurately and appropriately.C) Recognize and respond effectively to verbal, nonverbal, and written communication.D) Ability to follow established policies and procedures dealing with health care.E) Comply with established risk management and safety procedures.F) Able to establish and maintain effective working relationships with the public and health care team.G) Must respect the confidential nature of medical information.H) Working knowledge of standard concepts, practices, and procedures.I) Able to work efficiently and copy with emergency situations.J) Capable of using experience and judgment to plan and accomplish goals.K) Good computer skills and a working knowledge of Microsoft Office.L) Able to read and interpret documents such as charts of document inthe electronic health record if available, safety rules, operating and maintenance instructions, and procedure manuals.M) Ability to write routine reports and correspondence.N) Ability to stand for long periods of time.IV) Internal & External Customer ServiceA) All participating pharmacies, Health Care Providers in the community, Trinity Springs Pavilion, John Peter Smith Hospital, various private hospitals in the community, and Public Health Clinics.B) Must demonstrate compassion, trauma-informed care, and safety practices including suicide safe care.V) TravelA) Adhere to MHMR Mileage and Travel reimbursement policy and any other aspect regarding travel.VI) Equipment UsedA) Utilized as required for position. Minimum Requirements VII) Minimum QualificationsA) Minimum Education: Bachelor's DegreeB) Defined Education: Social work or human services field such as psychology, child developmentC) Preferences: Bilingual-SpanishD) Substitutions: NoneE) Years' Experience: NoneF) Defined Experience: NoneG) License/Certifications: Valid Texas Driver's LicenseH) Special Courses: NoneI) Supervisory Experience: NoneVIII) Agency RequirementsA) All staff are required to participate in agency Emergency Preparedness and Environmental Safety programs and may be assigned by their department as a key/essential staff level function during critical events or for the purpose of sustaining business continuity.B) This position may require temporary or permanent re-assignment to any MHMR Tarrant facility as determined by program needs and/or the Division/Director.C) Assigned work hours may change as the needs of the agency change.D) The Functional Title of this position may change as the needs of the agency change.E) All work will be completed within the scheduled work hours. All non-exempt (hourly) employees are expected to clock in and clock out for each work shift, no work should be done off the clock.F) MHMR reserves the right to change, add to or eliminate positions as it deems appropriate. G) Employment is at will, as well as agency needs may change.H) Agency dress code is to be followed at all times.I) Physical on-site presence, including regular attendance and punctuality, is an essential function of this position.
