Carry small caseload of 10-12 families. Facilitate High Fidelity Wraparound services: utilize a strengths-based approach to safety planning, development of family team and family support systems, and wraparound planning for the purpose of maintaining children in their homes, schools, and communities. Prepare Wraparound Plan with families and agency partners with measurable outcomes. Complete documentation in an electronic health record Become credentialed as a High-Fidelity Wraparound Facilitator within 12 months of employment. Ideal Candidate. Passionate about applying the principles of High-Fidelity Wraparound, including Family Voice & Choice, Team Based Planning, Natural Supports, Collaboration, Strengths Based approach, and more Experience providing services for children and families with complex needs.
