Health Information Management Analyst II
GENERAL SUMMARY:Assemble and analysis of all inpatient, surgical day patient, and observation patient medical records to ensure completeness of documentation. This analyst will assist with incoming department phone calls. He or she will also aid in the release of patient information to patients and providers in compliance with all federal and state statutes, and Speare Memorial Hospital policies. Acting as the Birth Registrar, he or she will be responsible for the input of birth information into the State of New Hampshire system to compile birth certificates for the facility.
ESSENTIAL JOB FUNCTIONS*:- Assembles and completes analysis of inpatient, observation and outpatient charts for completeness of documentation; flags missing reports and signatures for completion by appropriate clinician. Follows up with appropriate ancillary departments to obtain missing reports, verifies patient name and medical record number appear on all pages.

- Works with all providers to ensure all charts are completed timely in the electronic medical record; monitors chart completion process and advises manager of deficiency trends and patterns.
- Retrieves in-house charts of all admission/discharges from nursing units on a daily basis
- Coordinates statistical reports for State and Federal regulatory agencies
- Works in conjunction with the HIM Manager to ensure all RAC / ADR requests are processed timely and efficiently. Assist with Front Desk coverage as needed.
- Assist with telephone coverage for incoming department calls and follows up on inquiries, requests, investigative issues, requests from other departments, and outside facilities.
- Processes birth certificates in accordance with State of NH Vital Records requirements.
- Assures that the Advance Directive documentation is appropriately filed, scanned into the medical record, and documented in hospital information system.
- Faxes medical information in accordance with department release of information procedures, as needed.
- May perform other data collection/clerical activities as needed.
- Complies with hospital and departmental policies, procedures, regulations and standards as related to job functions.

Qualifications: Required: High school grad or equivalent with completion of post- high school medical terminology course
Preferred: Post-secondary medical secretarial or health information management
Required: 2 years' experience in medical office or hospital medical records department.
Preferred: 3 - 5 years' experience in a medical records setting within a provider practice or hospital setting
Required: A demonstrated proficiency on computers and software and excellent customer service skills. Ability to perform highly detailed tasks for prolonged periods of time. Ability to organize and prioritize multiple tasks with minimal supervision. Ability to operate office equipment, including computers, fax machine, photocopier, telephone system, and printers.
Preferred: Knowledge of medical records confidentiality statutes and hospital policies and procedures governing release of patient information.
