Community Health Action of Staten Island (CHASI)drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community.
POSITION SUMMARY:
CHASIs Prevention Department focuses on seeking out and coordinating outreach, education and referrals for priority populations to address a broad range of needs to improve overall health and wellbeing and decrease acquisition of HIV and STIs through comprehensive effective strategies. At the core of the service delivery model, Navigators play a critical role in these efforts by aiming to address the barriers clients face in accessing the services they want and need. Navigators will provide case management and referrals services for clients in the target populations in order to provide comprehensive HIV prevention services. This role specifically involves Women UP! which is dedicated to supporting women of color.
This is a temporary full-time position (available for a 3 month period of time with possible 3 month extension thereafter) that generally works Monday-Friday, 9a-5p and requires a flexible schedule such that some of the job tasks occasionally, with notice, will be conducted during evening hours and on weekends.
DUTIES & RESPONSIBILITIES:
+ Conduct intakes, mental health, and SUD screenings and other assessments as needed to identify the full complement of needs for enrolled persons from priority populations.
+ Educate and counsel people on a variety of prevention education, strategies, and materials according to client presentation and needs.
+ Develop, coordinate, and integrate service plans in collaboration with clients.
+ Assist clients with appointment scheduling and provider follow ups; plan logistics, provide reminders, assist with transportation, and accompany clients to appointments as needed.
+ Perform and maintain effective case management for a case load of clients, as assigned, from intake to discharge.
+ Track and monitor client progress and maintain detailed, accurate, and timely notes.
+ Be familiar with internal and external services in order to provide appropriate referrals to clients; build relationships and work collaboratively with partner providers.
+ Provide clients with linkage, coordination, and referrals to and follow-up with appropriate ongoing providers, including PrEP, emergency PEP, iART/HIV care, STI treatment; ensure referrals are successful.
+ Work with clients to ensure connection and compliance with appointments and their agreed upon action plan and address barriers.
+ Collect and submit data to CHASIs Data Unit accurate data on persons contacted and engaged in services. Maintain updated and accurate records on the services and activities provided Participate in documentation quality assurance and reconciliation tasks.
+ Keep up to date on a weekly basis with the program client list and deliverable tracker
+ Participate in training, professional development activities, and meetings, as assigned.
+ Perform all functions in alignment with CHASIs Mission, Vision, and Core Values
+ Other duties as assigned OR below statement (dont need both)
+ Occasionally working nights and weekends.
Requirements
QUALIFICATIONS:
+ High School diploma or equivalent required.
+ Have basic knowledge regarding HIV, HCV, STI, PEP/PrEP and Harm Reduction methods.
+ Experience providing case management and/or health education with individuals at high risk of contracting HIV/AIDS, have a history of mental illness or substance use, immigrant or LGBTQ+ populations, etc.
+ Bilingual (English/Spanish) preferred
+ Experience in PrEP AP, Gilead, and applying for copay cards preferred.
+ Excellent MS Office skills (Outlook, Word, Excel, PowerPoint).
+ Ability to work independently and also be team player with an ability to work with diverse teams with shared responsibility and accountability; ability to contribute to a positive, productive, open, and supportive environment that motivates others to perform at their best.
+ Must possess ability to interact with all levels of external and internal partners with a high level of professionalism and tact in representing CHASI.
+ Strong verbal and written communication skills, including capability to listen and follow instructions; able to give, provide direction and follow up on tasks; high attention to detail and accuracy. Be highly organized and able to multitask
