Job Summary
We are seeking a Registered Nurse to join our Home Health team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems. Must have 2 years experiences as a home care or home health care Registered Nurse withing the last 5 years.
Responsibilities
Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
Administer medications and monitor patients for side effects or reactions
Coordinate/oversee/supervise the work of Home Health Aides, Certified Nurse Aides, and Personal Care Workers and provides written personal care instructions/care plan that reflect current plan of care. Monitor the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility
Prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, updates/summarizes patient records, and confers with other health care disciplines in providing optimum home care
Document any changes in patient conditions and report on changes
Maintain accurate records of patient care and discuss observations with the supervisor
Regularly communicate patient progress to the clinical manager and care team.
Plans and provides staff education.
Performs on-call responsibilities and on-call services to patients/families as assigned.
Participates in clinical development and continuing education programs.
Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.
Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members.
Completes documentation timely and accurately.
Regularly communicate patient progress to the clinical manager and care team.
Plans and provides staff education.
Participates in clinical development and continuing education programs.
Other duties as assigned.
Qualifications
Currently licensed Registered Nurse (RN) in state of employment
Must have 2 years experiences as a home care or home health care Registered Nurse withing the last 5 years
Excellent interpersonal and client care skills
Valid driver's license
First aid/CPR certification required
