Position Summary:
The Home Health Aide (HHA) will be responsible for providing personal care to patients, assisting with activities of daily living following a care plan developed by a registered nurse or a therapist. The HHA will have effective communication with patients and their families. The HHA will work closely with the patient's care team to provide seamless continuity of care. The HHA is responsible for following the aide care plan, reporting issues related to providing care and patient symptoms as directed in the care plan.
Education, License & Certification requirements:
PA/NY residents require High School Diploma or GED Required.
BLS required
PA residents require Current Pennsylvania Certified Nursing Assistant (CNA) certification.
NY residents require Completion of a NYS DOH approved certification program required. HHA must be in good standing with the NYS home care services worker registry.
Experience:
Minimum one year of relevant experience. Experience with public contact is preferred.
Essential Functions:
1. Updates Knowledge and documentation per agency standards
2. Understands and adheres to Guthrie's compliance standards as they appear in Guthrie's Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy.
3. Attends in?service programs designed to update knowledge and skill related to patient care and to keep abreast of all pertinent federal, state and Guthrie regulations, laws, and policies as they presently exist and as they change or are modified relevant to job.
4. Reads aide care plan at the beginning of each home visit and performs all those tasks listed on care plan.
5. If care plan tasks cannot be accomplished the aide is to notify the case manager and document in the medical record the tasks not completed and reasoning
6. Accurately, timely and completely document the visit in the medical record.
7. Observes infection control protocols per agency policy utilizing personal protective equipment provided by the agency.
8. Familiarize self and seek?out instructions as needed in relation to all patient care tasks and equipment.
9. Notifies the patient's case manager and a home health manager if unfamiliar with a patient care task and or equipment found in patient's home. The HHA must request an orientation to new or unfamiliar cases and tasks.
10. Reports any care issues and concerns to an agency manager promptly.
11. Respects the patient's home and environment and establishes a professional but friendly and supportive relationship with the patient.
12. Complete annual education requirement. State (PA and NY) and federal in-service education mandates include 12 hours per year for home health aides.

Pay Rate ranges from $16.00 - $23.85 per hour.
