Tallavera is currently seeking a Registered Nurse (RN) Home Care Case Manager in Rochester, NY. Preferred candidates for the Registered Nurse (RN) Home Care Case Manager position will be a New York State Licensed Registered Nurse, with 1 year experience as a clinical nurse in a hospital or other direct patient care clinical setting.*Responsibilities of the Registered Nurse (RN) Home Care Case Manager:** Assesses and charts observations of the patient's condition at each visit* Revises plan of treatments and discharge plan in conjunction with registered nurse case manager, patient, family, physician, care delivery team, and third-party payors including obtaining medical orders and approval of medical services.* Assures the completion of all appropriate clinical records needed for compliance with state and federal legislation and the delivery of seamless service within established timeframes.*Requirements of the Registered Nurse (RN) Home Care Case Manager:** NYS driver's license* NYS Licensed Registered Nurse* OASAS certification preferred\*NO COVID VACCINE REQUIRED\*#RN123Job Type: Full-timePay: Up to $55.00 per hourBenefits:* 401(k)* Dental insurance* Health insurance* Paid time off* Retirement plan* Vision insuranceSchedule:* 8 hour shift* Day shiftLicense/Certification:* RN License (Required)* Driver's License (Required)Work Location: In person
