Overview:
The Concierge performs non-certified, participant focused, non-clinical home care supports as directed by the interdisciplinary team. Services are often provided on a 1:1 basis and will be provided in the participant's home or in a contracted placement such as an Adult Foster Care, Assisted Living, or Skilled Nursing Facility.
What you'll do
* Provides 1: 1 supervision of assigned participants.
* Facilitates diversional activities and re-direction when a participant may need it.
* Conducts light home making, including dishes, sweeping, general tidying etc.
* Prepares meals for participants when necessary.
* Ride along with participants on the bus when needed. This may be to the participants home, PACE Center or an external visit.
* Provides coverage in the Day Center when requested.
What it Takes
* Must have reliable transportation and a valid MI driver's license.
* One year of experience working with the frail elderly, long term care population or end-of-life care preferred.
