Reporting to the Rehabilitation Team Lead, the Physical Therapist is responsible for comprehensive patient management including evaluation, diagnosis, and intervention, and patient/family education for development and implementation of individualized care planning in the home setting. Opportunities include working directly in a multidisciplinary fashion, including specialty care groups such as Parkinson's Disease, Covid Recovery and Post-Acute Stroke care. The ideal candidate demonstrates strong clinical skills in Physical Therapy, with opportunity for clinical growth through education, in-services, and care management groups. Minimum Qualifications
Education
Bachelors Degree or higher from an accredited physical therapy program.
Experience
Prior Home Care experience is preferred, or at least one year previous experience in an acute and/or rehab setting.
Licensure and/or Certification
MA PT license and BLS required within 30 days from hire.
