Job Description: Brief DescriptionThe Home Care Registered Nurse or Nurse Practitioner provides and directs the provision of home nursing care, based on agency policies and procedures, through the competent application of the nursing process. Collaborates closely with all disciplines to ensure patient needs are met in accordance with plan of care. Maintains clinical records in accordance with New York State and Federal regulations, and Home Care policy. The Home Care Registered Nurse or Nurse Practitioner supports organization's mission to provide the highest quality care by complying with regulatory guidelines, meeting agency standards, and obtaining appropriate certification.ResponsibilitiesJOB DUTIES / RESPONSIBILITIES: Provides Home Care nursing services, in accordance with the plan of treatment signed by the physician, and makes the initial evaluation visit to the patient as needed. Collaborates with team members, physicians and patients/caregivers to re-evaluate the nursing needs of the patient; initiates, develops, implements and makes necessary revisions to the patient's plan of care. Assesses the patient's continual care needs. Addresses all problems in the plan of care or documents rationale for not doing so. Completes MD orders and 60 day summary according to regulatory guidelines. Initiates diagnostic, preventive and rehabilitative nursing procedures as appropriate to the patient's care and safety. Makes referrals to other disciplines as indicated by the patient's needs and in accordance with physician's orders, or documents rationale for not doing so. Observes signs and symptoms and reports to the physician and/or other appropriate health professionals as often as needed, or upon changes in patient's condition. Teaches, supervises and counsels the patient and family regarding home nursing procedures and other care needs as appropriate to the patient's condition. Utilizes agency educational material as appropriate. Coordinates the total plan of care for all of the RN's caseload and maintains continuity of patient care by collaborating with other health professionals assigned to the same patients home care policy monthly and as necessary. Attends patient care conferences when needed or requested. Initiates patient care conferences for complex and/or multi-disciplinary patients whenever needed. Maintains collaborative relationship with Supervisors to ensure adequate coordination of care in accordance with Home Care policy. Supervises and teaches the home health aide every two weeks if required to complete a field visit. Assesses the patient's need for unskilled care and revises the plan of care as appropriate. Communicates the plan of care changes to the MD as often as necessary. Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Performs all types of nursing visits according to agency policies and procedures. Documents accurately and submits documentation in a timely fashion in accordance with program policy. Responsible for following all policies and procedures of the agency regarding service delivery, documentation and care coordination. During supervisory visits, exhibits full compliance and verbalizes knowledge of agency policies including, but not limited to, patient education, infection control, management of hazardous wastes and age-specific behaviors of patients. Demonstrates competency in all skills required for the agency, including expertise in pediatrics. Participates in on-call rotation. Performs ongoing record review to ensure quality documents and compliance with policies and regulations. Ensures signed MD orders are generated in accordance with policies and procedures no less than every 60 days and as needed. Meets established productivity standards. Participates in the agency's quality improvement program Participates in appropriate community activities as requested. Performs other duties as required.RequirementsFormal education required: Associates degree in nursing; B.S.N. preferred.Holds a valid Registered Nursing license or Nurse Practitioner license to practice in New York State.
