Job Description:
Responsibilities:
  • Providing confident, efficient, and complete care as outlined the individual care plan of resident; may require assistance with areas such as bathing and hygiene, meal preparation, medication reminders, laundry services, escort for shopping and errands, accompanying resident to medical appointments, pet care, assistance in going for a walk/exercising, light housekeeping, home management, and/or companionship
  • Maintaining educational requirements of 12 hours annually
  • Consistently maintaining registration required by state of PA
  • Maintaining annual CPR certification, which includes training on the use of an AED device
  • Establishes a compassionate environment by providing support with a friendly and caring approach to resident, friends, families, and staff in the residents home environment which may be independent living, personal care, and/or skilled nursing
  • Communicates resident well-being and needs by reporting concerns and/or changes in resident's physical, psychological, and social conditions to the home care manager
  • Maintains clean working environment by maintaining resident's living environment neat and orderly
  • Completing job list responsibilities on assigned shift
  • Adhering to infection control procedures
  • Maintaining appropriate personal appearance and hygiene
  • Documenting information in a timely and accurate manner in the resident's chart
  • Maintains continuity of personalized plan of care by communicating actions and needs of resident to the Resident Care Department
  • Accompanying residents (all areas) to medical or other appointments, in accordance with Kendal~Crosslands Communities policies transportation schedule or as assigned by supervisor
  • Ensuring that necessary paperwork is provides by nursing staff and brought to appointment, and then transferring instructions to nursing staff upon return
Qualifications/Experience:
  • Successful completion or an approved nursing assistant program and certification exam
  • Maintains state requirement annual education hours
  • At least one year of experience in a home care or long term care setting
  • Must possess current, valid PA driver's license in good standing and must be insurable by Kendal~Crosslands Communities Insurance
  • Must have access to reliable personal vehicle to travel from home to home
Physical demands:
  • Stands/walks most of the shift
  • Must be able to bend, stoop, push, pull, lift, and transfer residents weighing 50-250 pounds
  • Able to wear gloves and other protective equipment
  • Capable of clearly communicating with residents
