We are investing in building out a best-in-class management team and differentiated technology to help support our continued expansion.SUMMARYThe full-time Home Care RN Case Manager will be responsible for planning, providing and coordinating quality Client care throughout New York. The Home Care RN Case Manager plays an integral part in providing and coordinating care, including assessing the patient's condition and providing them the necessary resources, options, and coordination for a healthy care outcome. This role also acts as an advocate for patients and their families linking them to other members of the care team. The Home Care RN Case Manager role will require both patient visits (~75%) and remote work (~25%).DUTIES AND RESPONSIBITITIES
- Performs the start of care assessment and responsible for creating the overall individualized plan of care for the care that will be delivered by other nursing and home health aide staff
- Initiates the Plan of Care and necessary revisions and updates to the care plan
- Serves as the primary point-of-contact for families and Clients for all clinical matters; conducts and directs meetings and calls with Clients and their families
- Coordinates with the Client Care team to prepare care givers and to ensure that the Plan of Care is being correctly executed
- Based off of the needs of the Clients will, at times, need to perform skilled nursing visits during normal business hours and outside of normal business hours
- Provides Client education and teachings as needed
- Participates in weekly Client Care Review meetings in the Westchester care center with other members of care team
- Will provide case management services as directed by the Director of Nursing
- Supervises home health aides as required by state regulations
- Ability to adapt quickly to schedule changes as the needs of Clients change
- This will be a hybrid position, roughly 75% field work in patient's homes and 25% remote.
REQUIREMENTS
- 2+ year experience in acute care setting required
- Experience in home health, home care and/or hospice required
- Registered nurse (RN) with active, unencumbered licensure to practice professional nursing in the state of New York
COMPENSATION The position will offer competitive compensation of $100,000 - $110,000 per year.
