The Home Care Social Worker facilitates the provision of supportive services to assist to meet the needs of the patient and caregivers, with a holistic approach, keeping with the patient and caregiver's beliefs and goals of care. Provides consultation and education for patients, caregivers, and the Home Care team. The Home Care Social Worker documents in the patient's medical records in a timely manner according to agency requirements. The Home Care Social Worker provides patient care based on provider's orders, departmental policies and procedures, regulatory guidelines and best practice. The Home Care Social Worker participates case conference and team meetings.Required Education:Master's degree in Social WorkRequired Skills:Generally accepted standards of practice in respective discipline. Professional and efficient verbal and written communication skills. Good interpersonal skills. Ability to multi-task, basic organizational and time management skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (copy machine, fax machine, etc.) Ability to be open, sensitive, flexible and ecumenical. Must have demonstrated experience in working with patients and caregivers dealing illness, social, emotional and economic needs. Must be capable of prioritizing and handling multiple tasks of critical importance simultaneouslyPreferred Skills:Experience with Home Care Home Base; Experience with smart phones and smart devicesRequired Licensure/Registration/Certification:Current Texas LMSW in good standing; Current Texas state driver's license; Current auto insurance. Must have reliable transportation.Preferred Licensure/Registration/Certification:Advanced education or certification in area of specialtyRequired Experience:Two years' experience in social work fieldPreferred Experience:Experience in respective discipline in a hospital or other healthcare setting; Experience in Case Management; Supervisory position in healthcare; Hospice or Home Care experience
