Overview The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals.
- 5 x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

This position is on site at St. Francis Hospital in Roslyn, NYResponsibilitiesOur Home Care Transition Nurses:
- Establish and maintain professional relationships with key physician groups and referral sources.
- Act as Liaison to referral sources on behalf of Catholic Home Care.
- Identify potential market growth opportunities and program expansion.
- Serve as a Customer service representative and act in a professional and supportive manner at all times.
Requirements: Registered Nurse with current licensure in NYS
- 2 or more years of home care experience preferred
- Customer Service and Marketing skills required
Pay Range USD $90,000.00 - USD $105,000.00 /Yr.
