Responsibilities
The Virtual Wound Care Registered Nurse plays a key role in the Home Health Virtual Wound Program, utilizing advanced telemedicine technologies to deliver expert wound care remotely. This position requires an experienced professional that is skilled in complex wound assessment, management, documentation, care planning, and product selection to promote optimal wound healing for home health patients. The clinician is responsible for conducting comprehensive virtual assessments of patients with wounds or integumentary system complications across the home health continuum. In some cases, the clinician may also provide in-home care consultations for complex care management. This role involves close collaboration with the nurse case manager and field clinicians to provide guidance and ensure high-quality patient care, accurate wound assessment, and proper documentation.
Conduct virtual assessments via Google Video Chat and the Rover application using practice expertise in wound, ostomy, continence management.
Ensure precise and complete documentation in compliance with policy, procedure and regulatory guidelines.
Demonstrates proficiency in selecting wound care products that enhance healing while effectively managing costs.
Demonstrates the ability to collaborate with clinical staff to update care plans, write orders, manage supplies as needed and ensure appropriate utilization of supplies within payor guidelines.
Participate in training sessions to enhance proficiency in approved technology platforms.
Maintain up-to-date knowledge of care standards and integrate them into nursing practice and share knowledge and best practices with clinical staff.
Contribute to the success of the Virtual Wound Program through collaboration and feedback.
Conduct in-home consultations and/or supervisory visits with nursing staff to ensure all quality indicators are met, including proper wound care assessment, pain management, and infection control practices.
Conduct chart audits to ensure continuous survey-ready documentation in the EMR.
Collaborate with agency leadership and staff development to support wound care education and training.
Develop and update patient-specific care plans and ensure documentation complies with physician orders and reflects progress toward goals.
Act as a liaison between the multidisciplinary team to ensure the effective coordination of care.
Participates in performance improvement projects by collecting data, analyzing results, and developing action plans to improve quality of care.

Lifts a minimum of 50 lbs, pushes and pulls a minimum of 50 lbs and stands a minimum of 8 hours per day.
Qualifications
Education, Knowledge, Skills and Abilities Required:
Bachelor of Science in Nursing (BSN) degree.
Minimum of 1 year of experience in wound care, with a focus on complex wound management.
Proficiency in PC skills and technology.
Excellent written and verbal communication skills.
Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
Education, Knowledge, Skills and Abilities Preferred:
Licenses and Certifications Required:
NJ State Professional Registered Nurse License.
AHA Basic Health Care Life Support HCP Certification.
Certified Wound Specialist or Wound Care Certified or Certified Wound Care Nurse.
