If so, we want to meet you. ***This is a PRN position*** Dover Home Health and Private Duty, a regional healthcare provider and true friend of adults facing terminal illnesses, seeks a service-minded, career-oriented individual to join our team as a Home Care Aide at our Joliet, Il location. As a Home Care Aide, you will have the opportunity to connect with, and positively impact the lives of, our patients daily! You will promote the physical, personal, and emotional well-being of patients and work with the Dover Health teams to strive to reach the ultimate goal, which is to return the patient to their maximum level of self-care and independence, when possible. As a Home Care Aide, you will impact the lives of our patients by -

  • Working in collaboration with the supervising RN, PT or OT to fulfill the defined patient-specific Aide Plan of Care.
  • Collaborating with the care team as an extension of nursing or therapy services to ensure all patient needs are fully addressed.
  • Delivering hands-on personal care and supportive services to assist with safe activities of daily living, in accordance with the established Aide Plan of Care, and in compliance with the applicable laws, regulations, and agency policies.
  • Documenting care provided according to agency policy.
  • Using Durable Medical Equipment safely.
  • Participating in activities that promote patient safety, quality, and regulatory compliance.
  • Participating in professional development and lifelong learning activities.
  • Performing other responsibilities as assigned
To become a Home Care Aide, you will need -
  • Current Certified Nurse Aide (CNA) certification
  • Current Basic Life Support/CPR certification
  • Current and valid driver's license, reliable transportation, and current liability auto insurance.
  • Must be able to meet all physical demands of the job which includes, but is not limited to seeing, speaking, touching, walking, standing, bending, reaching overhead, crouching/kneeling or crawling, as well as the ability to regularly lift/move up to 50lbs independently and up to 200lbs with assistance.
  • Ability to work well with others and take direction from management, as well as take initiative - willing to go above and beyond to ensure our patients needs are met and they are comfortable in their living environment.
  • Self-motivated to accomplish identified goals with a strong sense of accountability for results.
