Job Summary: The employee escorts residents to and from clinic appointments and dialysis sessions. They must arrive one hour early, dressed in full nursing uniform with an ID badge. Responsibilities include assisting residents, providing snacks, and ensuring their safety during the appointments. Reporting incidents promptly and acting as a facility representative are essential. They may have multiple clinic visits per day and report to the clinic coordinator or ADNS directly.
Pay Rate: Up to $16/hr
Duties and Responsibilities
- Escorting Residents to and from clinic appointment/ Dialysis
- The employee will be responsible for arriving at work one (1) hour prior to scheduled clinic appointment.
- The employee must be in full nursing uniform (color as designated) at all times with ID badge on in facility and while in clinic with the Resident.
- Assist in getting the Resident ready for his/her appointment.
- Ensure that the Resident receives their out on pass snacks.
- Monitor residents to prevent falls and altercations between residents.
- The employee is responsible for intervening immediately to prevent incidents.
- The employee is responsible for reporting all incidents and accidents to the clinical coordinator or ADNS immediately.
- At the clinic, the HHA is responsible for obtaining appropriate report regarding the resident, type of visit and ensuring he/she has the appropriate paper work and supplies to accompany the resident.
- The HHA must know residents DNR/DNI status before departing, ensuring all DNRs have a non-hospital DNR form with them.
- The HHA is to be pro-active on behalf of the resident while at clinic to ensure all needs are met. The HHA must immediately call the facility and speak to the clinic coordinator regarding any challenges encountered while at clinic appointment.
- Position may require more than one clinic visit per day, depending on needs of the facility.
