Key Responsibilities:
- Escort residents to and from clinic appointments, including dialysis.
- Arrive at work one hour before scheduled appointments to assist residents with preparation.
- Maintain a professional appearance, including wearing the nursing uniform and ID badge, at both the facility and clinic.
- Assist with pre-appointment needs and ensure residents receive snacks for the outing.
- Monitor residents closely to prevent falls, ensuring a safe environment and intervening as needed.
- Promptly report any incidents or accidents to the Clinical Coordinator or ADNS.
- At the clinic, gather relevant reports and paperwork, ensuring DNR/DNI forms accompany the resident.
- Act as an advocate for residents, addressing any issues by contacting the Clinic Coordinator.
- Be prepared for multiple clinic visits per day, depending on facility needs.
Requirements:
- Certification: Current New York State Home Health Aide (HHA) certification.
- Experience: Proven ability to fulfill all Home Health Aide duties.
- Skills: Strong communication skills, especially in relaying residents' conditions accurately.
- Attendance: Punctuality and commitment to perfect attendance are essential.
