Position Summary: As a Home Health Aide II, you will provide essential care to patients as outlined in individualized care plans developed by our RN Case Manager. Your role includes assisting patients with daily living activities under the guidance of an RN, contributing to high-quality patient care, and being eligible to provide hospice care in skilled nursing facilities.

Education and Certification: A high school diploma or GED is required. Candidates must possess a valid Home Health Aide certificate or CNA certification registered with the Connecticut State Nurse Aide Registry. Completion of a Basic Approved Training Program or meeting exemption criteria as defined by Licensure Regulations is necessary.

Experience: Prior experience as a CNA or HCA is preferred.

Key Competencies: You should be able to communicate effectively in English, follow written and verbal instructions, work collaboratively with team members, and show empathy towards patients and their families. A valid driver's license, proof of automobile insurance, punctuality, and reliability are essential.

Essential Duties and Responsibilities:

  • Assist patients with personal care and activities of daily living including bathing, dressing, feeding, and mobility.
  • Perform household tasks essential to patient care such as meal preparation and light housekeeping.
  • Report any changes in the patient's condition to the RN Case Manager.
  • Communicate effectively with patients and families at their level of understanding.
  • Maintain confidentiality and adhere to our Agency's corporate compliance and HIPAA privacy policies.
  • Participate in ongoing professional development and meet annual in-service education requirements.
  • Performance Criteria: You will be evaluated based on your competency, ability to provide safe and effective care, respect for the patient's rights, sound judgment in recognizing changes in patient conditions, and your professionalism at all times.
