Responsibilities:

The primary purpose of the Home Health Aide is to perform duties related to personal care and comfort of the residents according to their case management needs. Performs services necessary for the cleanliness and care of the residents/patients environment, personal items and equipment. Treats the residents/patients with respect and dignity, recognizing individual needs and encourages independence.

  • Report all accidents and incidents observed immediately.
  • Use only authorized abbreviations established by this facility when recording information.
  • Report all changes in resident's/patients condition to the supervisor.
  • Other duties/responsibilities within the scope of training as may be assigned to promote a high level of competent care and quality of life.
  • Ensure the resident's apartment is ready for receiving the resident (i.e., bed made, dishes done, laundry put away.)
  • Greet and escort the resident to his/her apartment.
  • Introduce residents to other residents and personnel as appropriate.
  • Make residents comfortable.
  • Assist residents in the care of their personal belongings as necessary.
  • Follow work assignments, and/or work schedules in completing and performing assigned work tasks.
  • Perform all assigned tasks in accordance with established policies and procedures and as instructed by the supervisor.
  • Notify the Supervisor when you will be late or absent from work.
  • Promptly report all complaints and grievances made by the resident to the supervisor or Administrator.
  • Cooperate with personnel from other departments.
  • Follow policies and procedures including dress code guidelines for Home Health Aides.
  • Be knowledgeable of and follow all Valley Residential Services policies and procedures.
  • Remain awake and alert for the entire working shift.
  • Assist residents/patients with activities of daily living and may include the following:
  • Assist with the care of teeth and mouth.
  • Assist with the grooming/care of hair including shampoo, shaving and ordinary hand care.
  • Assist the resident/patient with bathing in tub or shower.
  • Assist resident/patient on and off toilet as per individual care plan.
  • Assist resident/patient in transferring, wheelchair mobility, or ambulation as per Individual Service Plan.
  • Assist resident/patient with dressing.
  • Assist with managing incontinence of bladder/bowel.
  • Taking vital signs on residents/patients as directed including temperature, blood pressure, pulse, and respirations.
  • Assisting with use of ace bandages, TED stockings.
  • Collecting specimens (routine urine, stool, sputum).
  • Assisting with passive Range of Motion.
  • Assist with use of oxygen tank, concentrator, liquid oxygen reservoir, and nebulizers.
  • Assist with medications.
  • Administer eye drops.
  • Assist with meals and snacks.
  • Initiate and participate in leisure activities as described in the assisted living facility monthly activity calendar. Encourage resident socialization and participation in activities.
  • Make and change the bed.
  • Tidy bedroom, living room, kitchen and bath.
  • Collect and dispose of the resident trash.
  • Clean refrigerator and food inspections as assigned.
  • Clean resident equipment as assigned.
  • Clean residents personal laundry on designated days and put away.
  • Complete all household chores according to cleaning schedule.
  • Respond promptly and positively to resident requests for assistance, including emergency calls, telephone calls and requests from family and friends.
  • Follow proper procedures in emergency situations.
  • Know location of each resident in their assignment, reminding them to sign out when they leave the building. Notify the Supervisor or Administrator if you are unable to locate a resident.
  • List needed supplies and shopping needs for residents and give to Supervisor, Director of Clinical Services or Administrator/Case Management Director.
  • Provide assistance with physical and emotional needs. This may include:
  • Assist with the use of devices geared to disability as aids to daily living.
  • Assist resident with prescribed exercises as taught by appropriate professional.
  • Assist with prescribed skin care.
  • First Aid.
  • Help residents/patient with household skills.
  • Accompany resident to obtain medical care in other than your own vehicle.
  • The omission of specific statements of duties does not exclude them from the position if the work is similar, related or is logical assignment to the position.
  • Attend and participate in scheduled training and educational classes including mandatory in-services. Twelve (12) hours of in-service education per year is required by NYS regulations for Home Health Aides.
  • Attend and participate in scheduled orientation programs and activities.
  • Follow established safety precautions in the performance of all duties.
  • Report all safety violations.
  • Participate in all fire safety and disaster preparedness drills in a safe and professional manner. Know your responsibility in the event of fire or disaster.
  • Know and practice established universal precautions and procedures.
  • Wash hands before and after performing any service for the resident.
  • Notify your supervisor of any resident leaving/missing from the facility.
  • Use only the equipment you have been trained to use.
  • Operate all equipment according to instruction.
  • Use only the equipment and supplies necessary to do the job. Do not be wasteful.
  • Report defective equipment to your supervisor.
  • Inform your supervisor of your equipment and supply needs.
  • Follow established preventive maintenance procedures for maintaining your equipment and supplies in a safe and operable manner.
  • Maintain the confidentiality of all resident/patient care information.
  • Ensure you treat all residents/patients fairly, and with kindness, dignity, and respect.
  • Ensure all nursing care is provided in privacy.
  • Knock, wait for response, then announce yourself before entering the residents'/patients apartment.
  • Performs other pertinent care functions as assigned and demonstrated.
Qualifications:

Education:

  • High School Diploma or GED, preferred

License/Certification:

  • Successful completion of an approved training course to carry out assigned activities or have documented evidence of completion of an equivalency exam approved by NYSDOH, required

Physical Requirements:

  • Medium Work - Exerting up to 50 lbs. for force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects.
  • Occasional balancing, climbing, crouching, typing/clerical/dexterity, grasping, hearing, kneeling, lifting, pulling, reaching, repetitive motion, seeing/monitor/computer use, standing, stooping, talking, visual acuity, walking, fit testing

Hazards:

  • Occasional bodily fluids/bloodborne exposure and chemical

Travel:

  • Occasional Travel - The job may require travel from time- to-time, but not on a regular basis.
