JOB SUMMARY

The Home Health/Hospice Aide Certified Nursing Assistant provides personal care, home hygiene services and other supportive services to persons at their home or residence. This individual is responsible for accurate record keeping, documentation of the routine and delegate duties completed and notification to the Registered Nurse of any change in status. The Home Health/Hospice Aide CNA provides services and cares in accordance with a comprehensive patient care plan and under the supervision of a RN, physical therapist or occupational therapist.

JOB QUALIFICATIONS

EDUCATION

For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.

EXPERIENCE

Minimum Required: None

Preferred/Optional: Experience in hospital or nursing home.

CERTIFICATIONS/LICENSES

The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.

Minimum Required: Certified Nursing Assistant (CNA) certification awarded by the State of Wisconsin. Listed on the State of Wisconsin Registry as eligible to be employed at a state and federally -certified Home Health and Hospice agency. Basic Live Support (BLS) certification provided by the American Heart Association within three months of hire. Valid State of Wisconsin Driver's license with acceptable driving record.

Preferred/Optional: Certification in Hospice and Palliative Care awarded by the National Hospice and Palliative Care organization (NHPCO).
