We are seeking a kind and compassionate home health aide who has excellent communication and companionship skills to provide basic medical care, light housekeeping duties, personal care, and hygiene tasks so our patients are empowered to live happily at home. You must have 1-3 years of experience in home health implementing care plans, and make your patients health and safety your first priority. If this sounds like the perfect job for you, please apply today!Responsibilities: Enable patients to complete personal hygiene tasks like bathing, toileting, shaving, grooming, dressing, etc. Ambulate patients as indicated in the care plan, and help patients complete exercises and physical therapy as needed Complete basic medical tasks such as taking vital signs, measuring glucose and ketone levels, replacing bandages and dressings, and ensuring the proper use of medical devices Facilitate personal care tasks like scheduling appointments, calling doctors offices, coordinating Medicare and other insurance details Reinforce accurate and up-to-date care records and work with the case manager to implement the patient care plan Administer Medications: Dispense medications as directed by the Registered Nurse (RN), maintaining an annual error rate of no more than one mistake. Resident Response: Attend to resident call lights within 10 minutes to ensure responsive and attentive care. Light Housekeeping: Perform regular light housekeeping in resident rooms at least once weekly to promote a clean and comfortable living space. Service Charting: Accurately document all services provided without missing tasks, ensuring compliance with company standards.Qualifications: Some college preferred, but a high school diploma or GED is required A valid state drivers license, reliable transportation, and current auto insurance is required Ability to lift up to 100 lbs, you may be required to stoop, bend, reach, and transfer patients up to 250 lbs as needed Completion of home health aide training programs from community colleges are acceptable - Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA) is a plus 1-3 years of experience as an HHA/ home health aide, personal caregiver, nursing assistant, or personal care aid for a home health agency, at patient homes or care facilities settings is needed Extreme Ownership: Demonstrates accountability, ownership of outcomes, and proactively removes obstacles.
