WHAT YOU CAN EXPECT- Assists patients with care and activities of daily living as directed in the established Plan of Care.
- Completes necessary documentation of care provided. Records intake, output, and vital signs as directed and communicates care as appropriate.
- Provides assistance with personal hygiene and elimination needs according to the established plan of care.
- Assists patients with ambulation and transfer using proper body mechanics.
- Communicates any change in condition or lack of patient progress.
- Schedules visits with the patient or caregiver.
QUALIFICTIONS - High School Diploma/GED - Required
- 1 year Home Healthcare - Preferred
- 1 year Long Term Care Setting - Preferred
- Certified Nursing Assistant (CNA) OR Home Health Aide (HHA) - Required
- Cardiopulmonary Resuscitation (CPR) - Upon Hire- Required
- Auto Insurance - Upon Hire- Required
- Driver's License - Upon Hire- Required
