Job Summary: A Health Aide is a trained worker who, under the supervision of a registered professional nurse provides homemaking services and personal care services for an acutely or chronically ill client. Duties of the Home Health Aide are planned, assigned in writing and supervised regularly by the Home Health Aide Supervisor. Essential Job Functions and Responsibilities:Maintains consistently high quality client care
- Demonstrates compassion and maturity necessary for establishing and maintaining a good working relationship with the client.
- Flexibility in adapting to varied client needs and lifestyles.
- Assists the client with personal care activities, including bathing, dressing, oral hygiene, hair/skin care and feeding.
- Medication compliance: may assist by bringing medication to the client or remind the client to take medication.
- Simple treatment/dressing after demonstration by the Primary Care Nurse and re-demonstration by the Home Health Aide.
- Must be able to read and record temperature, pulse, respiration, and weight.
- Reports any changes in client's condition immediately to the Primary Care Nurse or Supervisor of Clinical Services.
- Assists the client with exercises, ambulation and transfer activities.
- Performs normal household services essential to client care at home, including meal preparation, laundry and housekeeping.
- Provides basic emotional and psychological support to the client and other members of the household.
- Provides companionship to the client.
- Maintains and follows Universal Precautions.
- Accepts assignment of duties as deemed necessary by the Home Health Aide Supervisor or all other clinical supervisors.
Follows policies and procedures
- Documents care provided according to plan of care and submits in timely fashion.
- Communicates changes in patient status to nurse or clinical supervisor.
- Maintains timely and proper charting records and time sheets.
- Adheres to appropriate dress code.
Completes educational and mandatory training
- Attends/receives a minimum of twelve (12) hours of continuing education/in-service education per calendar year.
- Participates in designated committees as assigned.
- Assumes the responsibility of knowing Bristol Hospital Home Care policies.
QualificationsEducational / Minimum Requirements: High School Graduate. Successful completion of a CNA or HHA training program of not less than 75 hours and approved by the State of Connecticut Health Services. Experience as CNA or HHA preferred. Maintains 12 hours in-service annually. State/Federal Mandated Licensure or Certification Requirements: State of Connecticut C.N.A. licensure. Bristol Hospital Mandated Educational Requirements: General orientation at time of hire. Fire/Safety/Infection Control annually. Other programs as mandated by Hospital. Successful completion of the agency's orientation program. Satisfactory rating on the agency's Standardized Home Health Aide Competency Evaluation. Special Requirements: Must possess interest in and sympathetic attitude toward caring for the elderly. Must maintain current, valid driver's license in good standing and automobile liability insurance in the amount of $100,000 to $300,000. Physical Requirements: Able to perform tasks involving physical activity, lift 50 pounds and transfer 150 pounds using proper body mechanics, extensive bending and standing, and climbing stairs. Must be able to endure prolonged periods of extended walking and stooping. Moderate periods of sitting and reaching. Occasional crouching, kneeling and balancing. Vision, hearing, smell and tactile sensation corrected or accommodated to normal. Work Environment: Regular contact with clients, families, body substances, and home environments. Use of various types of lifting devices. Cognitive Requirements: Must have the ability to understand and carry out directives and instruction. Ability to read, write and communicate verbally. Must possess the ability to read, write and speak English.
