Position Summary:The Home Health Aide reports to the Clinical Manager and is expected to function as a member of the health services team. The Home Health Aide is trained to provide personal care and related services in the home. The Home Health Aide is assigned to specific patients and functions under the direction, instruction, and supervision of the Staff Registered Nurse/Case Manager, the Supervising Therapist, and Clinical Manager. The Home Health Aide position is non-exempt.Required Experience for Job Title:
  • Currently Certified as a Home Health Aide in the State of Michigan.
  • Successful completion of certified training program with competencies including classroom and supervised practical training totaling a minimum of 75 hours.
  • Minimum of 1 year of Home Health Aide experience in a health care or home care setting.
  • CPR certification preferred, but not required.
  • Current driver's license with current automobile insurance and must have reliable transportation.
  • Must have current TB, Chest X-ray, or Skin Test (within last 12 months).
  • Excellent communications skills.
  • Compliant with accepted standards and practices.
  • Must be able to meet the physical demands of providing patient care, ambulating patients, transferring patients, assisting with exercises, etc.
  • Self-directed with the ability to work with little supervision.
  • Avid team player.
  • Consistently maintains a positive attitude which promotes team and optimal performance.
  • Must be empathetic, nonjudgmental, tactful, responsible, adaptable, organized, and committed to the purpose and objectives of the Agency.
Accountability for Home Health Aide:By following the Home Health Aide Care plan and/or assignment form, which has been completed by Registered Nurse or other appropriate skilled professional, the aide provides services ordered by the physician, included in the Plan of Care, permitted to be performed under state law, and consistent with Home Health Aide training including the following:
  • Assist in maintaining a clean, healthful, and safe environment.
  • Assist the patient with ambulation. Uses safe transfer and ambulation techniques.
  • Assist with nourishment and fluid needs including prepared meals, feeding patient, and offering fluids.
  • Reading and recording temperature, pulse, and respiration.
  • Maintains basic infection control procedures.
  • Adherence to the Home Health Aide care plan.
  • Recognizes emergencies with knowledge of instituting emergency procedures and their applications.
  • Encourages the patient to become as independent as possible according to the nursing care plan.
  • Gives simple emotional and psychological support to the patient and other members of the household.
  • Establishes a relationship with patient and family, which transmits trust and confidentiality.
  • Adheres to privacy and security standards provided by all federal, state, and local authorities of the Health Insurance Portability and Accountability Act (HIPAA) and other applicable state laws.
  • Works with personnel of other comprehensive agencies involved in the patient's care as directed by the staff nurse.
  • Communication skills, including the ability to read, write and verbally report clinical information to patients, representatives, and care givers, as well as to other HHA staff.
  • Will be considered a participating member of the interdisciplinary team and be a part of care conference.
Responsibility for Home Health Aide:
  • Performs all functions outlined in the Home Health Aide Assignment Sheet/POC.
  • Performs or assists the patient with personal/hands on care including bathing, hair care, dressing, shaving, oral care, etc.
  • Assists patient with positioning, transfers, ambulation, and exercises.
  • Performs or assists patient with bowel/bladder care including use of bedpan, perineal care, enemas, suppositories or digital stimulation as ordered, catheter care (not inserting or irrigation), colostomy care (not irrigation), bowel/bladder training programs, collections of specimens, and recording intake and output.
  • Provides care, treatment, and services in accordance with standards of practice, practice guidelines, law, and regulations.
  • Appropriate and safe techniques in performing personal hygiene and grooming tasks. Gives the patient personal care, including assistance in the activities of daily living including hair shampooing in bed and recognizing and reporting changes in skin condition.
  • Recognize and report changes in patient skin condition (including pressure ulcers) to supervising professional (Staff Registered Nurse, Clinical Manager, and other appropriate skilled professional).
  • Performs or assists with treatment procedures including prevention of decubiti, warm and cool applications, simple unsterile dressing changes, and reinforcing other dressings.
  • Takes and records temperature, pulse, respiration, blood pressure, and weight according to Home Health Aide Care Plan.
  • Prepares and maintains clinical documentation according to Agency policies and acceptable nursing standards.
  • Reports cases of possible abuse, neglect, fraud, non-compliance, and exploitation to the Compliance Officer, Administrator, or Designee immediately.
  • Plans for the patient's discharge from service.
  • Informs the Case Manager/Clinical Manager of patient services provided and needed and any changes in the Plan of Care.
  • Prepares clinical documentation of services provided and submits in a timely manner according to Agency policy.
  • Reports any change in body functions and/or mental status in the home situation and the care or service provided to her immediate supervisor, Staff Registered Nurse, Clinical Manager, or other appropriate skilled professional.
  • Carries out assignments as instructed by the nurse and reports to the nurse/therapist when able to do so.
  • Participates in orientation, in-service programs, staff meetings, and case conferences.
  • Participates in Agency Emergency Plan and Emergency Communication Plan.
  • Participates in Agency QAPI Program, effectiveness of which relies on involving the contribution of all skilled professional staff for their input and personal investment in the implementation of the program. Skilled professional staff, regardless of whether the staff is a direct employee of the Agency or under contract, are expected to contribute to all phases of the QAPI program.
  • Recognizes responsibilities involved in patient care as it related to patient rights and property.
  • Contacts the Clinical Manager to inform him/her of alternate schedule for patient visits during vacation, sick days, or personal days off. Reports any schedule change or inability to complete assignment as scheduled.
  • Accepts changes/additions to schedule during staffing difficulties when necessary. Accepts assignment in time of emergency, regardless of any location and/or any day of the week be it weekend/holidays.
  • Compliance with all applicable laws, regulatory requirements, standards of practice, accreditation standards, and policies and procedures.
  • Organizes daily visit schedule to minimize travel time and mileage. Maintains productivity according to work status.
  • Maintains OSHA and Infection control per policy.
  • Performs other duties as assigned.
