Synergy Medical Services is looking for compassionate members to add to our care team. Our caregivers aide our patients in the comforts of their homes by making sure they have the best quality of life possible by promoting and maintaining their independence. Our Home Health Aides provide personal care and related services in the home, under the direction, instruction, and supervision of the Supervising Nurse and as ordered by the physician. They provide hands on personal care, performing simple procedures as an extension of therapy and/or nursing services, such as assisting in ambulation, exercise and personal care.We are looking for caregivers to help service patients in our Region A2 location which includes: Gulf Breeze, Navarre, Pace and Milton and surrounding areas.Our Caregivers Responsibilities include the following:
- Following the patients plan of care for completion of tasks assigned to a home health aide by the registered nurse or other appropriate skilled professional to provide, safe, competent care/service to the patient.
- Help the client to maintain good personal hygiene and assists in maintaining a healthful, safe environment.
- Plan and prepare nutritious meals.
- Assist the client with ambulation as approved.
- Encourage the client to become as independent as possible according to the care plan.
- Maintaining an open communication process with the patient, representative (if any), caregivers, and family.
- Gives simple emotional and psychological support to the client and other members of the household and establishes a relationship with client and family which transmit trust and confidentiality.
- Honors the patient rights.
- Observe and reports any changes in the client's mental or physical condition or in the home situation to the supervisor.
- Perform routine housekeeping tasks as related to a safe and comfortable environment for the client.
- Demonstrates competency with assigned tasks.
- Complies with infection prevention and control policies and procedures.
- Prepare a visit report promptly and incorporate same in the client record weekly.
- Confirm on a weekly basis, the scheduling of visits so that other necessary visits by staff members can be coordinated.
- Work with personnel of other community agencies involved in the client's care.
- Attend in-service as required by regulation.
- Confer with the Supervisor about clients status and care, participate in the planning of service.
- Promptly report any change in client condition to the Supervisor.
- Works within the Home Aides scope of practice, as directed by a Registered Nurse or other appropriate skilled professional and reports in writing to the Supervising Nurse.
Qualifications:
- High School Diploma or equivalent
- CPR Certification
- Level 2 Background Check
- Must be able to pass Criminal Background Check, National Sex Offender Registry clearance and OIG clearance.
- Possess and maintain a valid Drivers License and Automobile Insurance
- Reliable Transportation
- Able to pass 75-hour FL HHA Certification Course or - proof of certificate, if already completed.
- 1 Year of previous experience providing personal/direct patient care
