The HHA Supervisor is a professional member of the StayingWELL team who provides assessment visits in clients homes, develops appropriate plan of care for clients based on individual needs, and provides supervision for Aides as needed/required by Agency policy and/or state/federal regulations. Our HHA Supervisor has a unique opportunity to ensure all patients are receiving the best possible care from our caregivers.

General Responsibilities: Meet with new clients as appropriate to review agreement and establish care plan Review and update care plans as needed (at least every 3 months) based on client change of conditions and/or needs Determine and communicate with Client Services Coordinators the skills/criteria necessary to ensure a positive Client/Caregiver match Communicate changes in care plans/client needs with clients and/or family members Receives and acts to resolve complaints made by patients/families regarding care or services received. Identify and manage high risk clients, refer to home health and hospice as needed Provide exceptional customer service to both clients and caregiver staff by responding to issues and/or requests in a timely, professional manner Actively develops and pursues referral sources, as well as facilitates on-going relationships with various ALF s Covers for other HHA Supervisors in their absence Perform other duties as requested by Director Knowledge of RVNAhealth s programs and services; makes referrals when needed to other service lines

Management Responsibilities: Provide new hire orientation and competency testing on a rotational basis Assess and update orientation agenda as needed per changing regulations and/or RVNAhealth policies Provide and/or assist with monthly aide in-services/continuing education programs Provide in person or phone orientation to caregivers on first time visit with clients Carry out disciplinary action for caregivers as needed Provide routine supervision of aide staff in clients homes

Qualifications: Current RN/LPN license in the State of Connecticut Private Duty: 1 year preferred Management Experience Must have reliable transportation Must be organized and detail oriented Able to communicate clearly with patients, families, visitors, leadership and others. Able to teach patients and families in accordance with the nursing plan of care. Knowledgeable about reimbursement sources, Medicare regulations, licensing laws, and accreditation standards Competent computer skills Current CPR certification Must be able to travel to, enter and function in the homes/facility of any patient to assess and provide care to all patients
