Role and ResponsibilitiesTrain to provide personal care and supportive services to individuals in the community, in the privacy of their home.Essential FunctionsTrain to provide health-related services as outlined in the plan of care, authorized by a physician, and established by a registered nurse or licensed therapist, under the direction of the Aide Matrix established by the NYS Department of Health.Train to provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home.Train to be able to complete and submit necessary patient documentation including charting tasks and observations on android device.All other duties as assigned.This job description reflects managements assignment of essential functions; and nothing in this herein restricts managements right to assign or reassign duties and responsibilities to this job at any time.
