DescriptionJOB SUMMARY
- The Home Health Clinical Intake Specialist will facilitate and assists in all needed information to properly schedule new patient referral. The Clinical Intake Specialist optimizes patient care by processing all aspects of outpatient referral requests from outside facilities. Compiles all pertinent patient records for initial consultative visit including but not limited to, H&P, relevant laboratory and diagnostic testing, pathology reports, and other data in accordance with physician preferences and disease state profiles.
QualificationsEDUCATION REQUIREMENTS
- 2 year / Associate Degree in Nursing (Required)
- 4 year / Bachelor's Degree in Nursing (Preferred)
EXPERIENCE REQUIREMENTS
- 4+ years RN Experience (Required)
- 4+ years Home Health Intake (Preferred)
- 4+ years Home Health Management or Market Analytics (Preferred)
- 4+ years Home Health Business Development
CERTIFICATIONS AND LICENSURES
- Required Certifications/Licensures: Registered Nurse (RN) with current Georgia license, Basic Life Support (BLS)
