The Home Care Coordinator is responsible for managing and overseeing the day-to-day operations of home care services, including coordinating care plans, scheduling caregivers, ensuring compliance with regulations, and maintaining positive relationships with clients and their families. The coordinator ensures that patients receive optimal care, tailored to their individual needs, while supporting caregivers in providing excellent service.

Key Responsibilities:

Care Coordination:

  • Efficiently schedule caregivers for shifts, ensuring coverage and avoiding scheduling conflicts.

  • Coordinate and monitor the availability of medical equipment, supplies, and other resources needed for client care.

  • Handle any emergencies or last-minute changes to the care schedule.

  • Develop and implement individualized care plans based on client needs, medical histories, and family preferences.

  • Keep detailed and accurate records of client care, schedules, staffing, and client communications.

  • Monitor client progress and ensure that care services are delivered according to the care plan within Alayacare and HHA Exchange.

  • Communicate with clients, families, and healthcare professionals to assess needs and discuss any changes in care.

  • Coordinate the scheduling of home caregivers, ensuring appropriate staff is assigned based on client needs and preferences.

  • Ensure caregivers are properly trained in specific client care needs and industry standards.

  • Provide ongoing support and guidance to caregivers, including offering feedback and addressing performance concerns.

    Quality Assurance and Compliance:

    • Ensure that all care services comply with relevant healthcare laws, industry regulations, and company policies.

    • Conduct regular visits or calls to client homes to assess care quality and satisfaction.

    • Monitor and evaluate client care outcomes, reporting any issues or concerns to supervisors.

    • Maintain accurate records of client care, including documentation of services provided, care plans, and incidents.

      Qualifications:

      • Education:

        • Associate's degree w/3-year experience within home healthcare (required).

        • 2 years Care coordinator experience or related fields is preferred.

      • Experience:

        • Strong understanding of home healthcare regulations and standards.

      • Skills:

        • Excellent communication and interpersonal skills.

        • Strong organizational and time-management skills.

        • Ability to work well under pressure and handle sensitive situations.

        • Knowledge of scheduling software, healthcare documentation, and other relevant tools.

        • Ability to supervise and motivate a team of caregivers.

      Working Conditions:
      • The role typically involves working in an office setting Monday through Friday 9-5pm, with occasional travel to clients' homes.

      • May require occasional evening, weekend, or on-call shifts to handle emergencies or urgent scheduling needs.

      Compensation:
      • Compensation typically includes a base salary paid weekly on Friday's.
