- Provide comprehensive nursing care based on the patient's care plan, including health and psychosocial evaluations, condition monitoring, health promotion, preventive care, service coordination, and direct nursing care.
- Conduct patient admissions, transfers, and discharges using OASIS assessments; prepare and present patient records for review.
- Collaborate with physicians and supervisors to update and implement patient care plans; submit clinical and progress notes weekly.
- Participate in case conferences, staff development meetings, and the patient's discharge planning process.
Patient Care Coordination:
- Conduct initial and ongoing comprehensive assessments, including Outcome and Assessment Information Set (OASIS) assessments at the appropriate time points.
- Develop and implement nursing care plans, explaining agency services to patients and families, and coordinating with other healthcare team members (e.g., Home Health Aides, therapists).
- Provide skilled nursing care as ordered by the physician, observing and reporting any changes in the patient's condition.
Supervision and Training:
- Supervise Home Health Aides and LPNs, conducting evaluations and providing guidance as needed.
- Participate in educational experiences for student nurses and continually strive to improve nursing skills through ongoing education and professional development.
Job Requirements:
- Valid driver's license and ability to drive to patient residences in Delaware County and North and South Philadelphia. (Company car available if needed).
- Ability to perform physical assessments and patient care, including lifting and turning patients.
- Graduate from an accredited School of Nursing, licensed as an RN in Pennsylvania, with at least 2 years of experience in community/home health or hospital settings.
- Knowledge of Medicare and Medicaid guidelines.
- Strong documentation skills with exceptional attention to detail.
- Prior experience with OASIS and proven expertise in documentation.
