Job Summary: We are seeking a compassionate and dedicated Physical Therapy Assistant (PTA) to join our team in providing high-quality, patient-focused care in a home health setting. The PTA will work under the direction of a licensed Physical Therapist (PT) to implement individualized treatment plans that help patients improve mobility, manage pain, and enhance overall physical function. This role requires direct patient interaction in a variety of home environments, offering a rewarding opportunity to make a difference in patients' lives.Key Responsibilities:Assist the Physical Therapist in implementing individualized treatment plans for patients in their homes, following specific instructions and goals outlined by the PT.Perform therapeutic exercises, manual therapy, and assist with modalities as directed by the PT.Educate patients and caregivers on home exercises, proper body mechanics, and strategies to improve functional independence.Monitor patient progress and document treatment outcomes, reporting any changes or concerns to the supervising PT.Ensure safe patient mobility and assist with transfers, gait training, and other mobility-related interventions.Maintain accurate, timely, and thorough documentation of patient care activities and progress in compliance with agency policies and regulatory requirements.Collaborate with other healthcare professionals, including nurses, social workers, and physicians, to ensure comprehensive and coordinated care.Adhere to all safety protocols, infection control measures, and maintain a clean and organized workspace.Qualifications:Associate's degree in Physical Therapy Assistant program from an accredited institution.Valid state PTA license.CPR and First Aid certification.Previous experience in home health, outpatient, or hospital settings is preferred.Strong communication and interpersonal skills to interact effectively with patients, caregivers, and healthcare teams.Ability to work independently and manage time effectively in a home health environment.Compassionate, patient-focused, and committed to improving quality of life for patients.Reliable transportation and ability to travel to patient homes as needed.Physical Requirements:Ability to lift, move, and assist patients (with assistance if necessary).Ability to stand, walk, bend, squat, and reach as part of patient care.Comfortable working in a variety of home settings.To Apply:Interested candidates should submit their resume and a cover letter outlining their qualifications for the position.
