Home Health Physical Therapist
We are a Medicare-certified home health and hospice agency that is seeking an experienced and caring full-time Home Health Physical Therapist for our Home Health division in Grants Pass and Medford, Oregon.
Compensation Package:
- $50+ Per Hour (Based on Experience)
- $7,500 Sign-on Bonus for Full-time!

Requirements:
- Clear and active Physical Therapist license in Oregon
- New graduates considered!
- 1+ Years of appropriate experience as a physical therapist. Community/home health experience is preferred
- Possesses and maintains current CPR certification
Responsibilities:
- Monday through Friday 7:30AM to 4:30PM
- Provide care to patients in the privacy of their home setting
- Using HCHB
- Working through admissions and routine visits to develop and carry out care plans
- Participating in interdisciplinary meetings to collaborate on approaches to patient care
- One-on-one time with your patients
