As a Home Health Physical Therapist, you will:
- Plan and administer prescribed skilled physical therapy treatment
- Test/screen the patient's physical strengths
- Develop and implement conditioning/rehabilitation programs
- Confer with physician and clinical team members
- Accurately document patients' care observations
- Report patient's progress to the physician and care management staff
Required Experience/Skills:
- Degree from an accredited Physical Therapy Program
- Minimum of one year physical therapy experience preferred
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Strong organizational and communication skills
Pay Range: $84,600 - $116,300 per year
