****What You'll Do
- Conduct patient assessments – Evaluate physical, mental, and social factors affecting patient health.
- Develop care plans – Collaborate with patients, caregivers, and the primary Medical Practitioner to create and adjust individualized care plans.
- Provide direct care – Perform tasks like wound care, infusion therapy, medication administration, and health education.
- Coordinate care – Guide and supervise LPNs and HHAs to ensure consistent, quality care for patients.
- Document and communicate – Keep accurate records of patient progress and report significant changes to the medical team.
****What You Need
- Active NYS RN License and a valid NYS Driver's License.
- Minimum of 1 year recent experience in geriatric care.
- Home care experience is a plus.
