Compass Healthcare Consulting & Placement is conducting a search for a bilingual Spanish, Home Healthcare Registered Nurse, RN, to visit patients at home for Patients in the Bronx and Upper Manhattan, NY. High Demand for Full Time Home Healthcare Field Registered Nurse. Field RN needed for Certified Home Care Agency. Experienced candidate in Certified Home Healthcare or will train new grad RN with some Hospital or Skilled Nursing Facility RN experience.

Qualified candidate will have a valid NYS RN License, prior Home Healthcare, CHHA Coordinator of Care (COC), Patient Service Manager (PSM) experience a plus, but will train newer RN's with some clinical background in Hospital or Skilled Nursing Home experience!

About the position:

The Field RN (Registered Nurse) performs home visits, assessments and follow up visits to patients. The RN advocates for quality patient care, collaborates with Physicians and other providers to maintain a safe and effective home environment for patients. The Registered Nurse works closely with patients, families and interdisciplinary team to ensure quality care is provided at home.

Qualifications:

• Must be a NYS Registered Nurse
• Knowledge of ICD 10 and OASIS Guidelines a plus
• Experience working for a CHHA or Prior Hospital or Skilled Nursing Facility experience
• Ability to visit patients in a home environment.
• Willingness to collaborate with Interdisciplinary Team as needed
• Excellent Organizational Skills Required.
• Ability to drive and have access to reliable automobile

This is a Full Time Direct Hire Position offering Benefits and Competitive Salary based on relevant RN and Home Health experience. Salary $115,000 to $125,000 plus excellent benefits!
