Your Responsibilities:
  • Patient Care:
    • Implement and manage individualized patient care plans.
    • Assess and document patient progress, ensuring compassionate and personalized care.
    • Support patients and families with guidance and reassurance throughout their care journey.
  • Coordination:
    • Assist in coordinating home health services with seamless communication between patients, families, and healthcare providers.
    • Collaborate with the healthcare team to deliver comprehensive, high-quality care.
  • Self-Care Education:
    • Educate patients and families on self-care strategies, empowering them to manage their health effectively at home.
What We're Looking For:
  • Licensing:
    • Valid RN License from the Nevada State Board of Nursing.
    • Valid Nevada Driver's License.
  • Certifications:
    • Current CPR and First Aid Certification.
    • Ability to obtain a TB clearance health card meeting federal/state requirements.
  • Experience:
    • Prior home health experience is preferred but not required.
  • Education:
    • Graduation from an accredited nursing school program.
