Key Responsibilities
- Plan, organize, and direct home care services tailored to individual patient needs.
- Conduct thorough assessments of patients' healthcare requirements.
- Develop and implement personalized care plans in collaboration with care teams.
- Counsel patients and family members, providing education and healthcare instructions.
- Prepare clinical notes and communicate patient conditions to primary physicians.
What We're Looking For
- Licensure - Valid Arizona State Board of Nursing RN License.
- Experience - At least one year in home health care or hospice preferred.
- Transportation - Valid Arizona driver's license and reliable transportation.
- Certifications - Current CPR certification and negative TB test (assistance available).
- Background Clearance - Satisfactory fingerprint clearance per federal/state regulations.
- Bilingual Preferred - Conversational Spanish skills highly desired.
Education
- Associate's Degree in Nursing or higher.
