Your Tailor Made Senior Service is seeking Registered Nurses with prior OASIS/Medicare Certified Visits for Adult home health assignments in Dallas and close surrounding areas.ResponsibilitiesTo provide quality, comprehensive nursing care in the Patient's home as prescribed by the physician in the plan of careEnsures quality and safe delivery of home health care services in compliance with all applicable laws, rules, regulations and policiesPerforms nursing assessments and develops, implements and evaluates plans of care in conjunction with the physician, patient and caregivers per agency policy and procedureEvaluates the effectiveness of the plan of care, the patient's response to care and outcomes of care and revises the plan of care as neededInitiates appropriate preventive and rehabilitative nursing procedures following the physician's plan of careDocuments all communications with the patient, family, physician, CTM, pharmacy, other disciplines, and appropriate others as indicated on communication notesPrepares a written plan for the Home Health Aide to follow, if applicableResponsible for collaborating with the interdisciplinary team to develop the plan of care for the PatientManage nursing treatments and activities to meet the needs of each individual patientResponsible for evaluating utilization of patient services and patient progressReview evaluation with the care team, patient, family members, physician, and case manager as neededOther duties as assignedYour Tailor Made Senior Service Home Health Nurses are paid by the hour, not by the visitNurse will have a designated territory of coverage - We specialize in care for VeteransRequirements: Valid RN license in TexasOASIS/Medicare Certified Visits Experience Electronic Charting experience-We use Finale HealthDeveloped computer skillsMinimum of 1 year of recent home health nursing experience Driver's License and Vehicle with Current Auto InsuranceAmerican Heart Association BLS Valid U.S. Driver's LicenseProblem solving skillsExcellent Communication SkillsPositive, Professional attitudeCandidates must be able to perform all essential job functions with or without reasonable accommodation
