As a home health RN, you'll have the opportunity to deliver personalized care, conduct in-home assessments, and support patients through their recovery journey. What You'll Do:
- Conduct In-Home Assessments: Create tailored care plans to meet each patient's unique needs.
- Administer Care: Provide medications, wound care, and other treatments as prescribed.
- Educate & Empower: Guide patients and families on managing health conditions and medications.
- Collaborate: Work closely with therapists, social workers, and other healthcare professionals.
- Monitor & Adapt: Track patient progress and adjust care plans for optimal results.
- Document: Keep detailed and accurate patient records.
What You'll Need:
- Education: RN license with an Associate or Bachelor's degree in Nursing.
- Experience: 2+ years of nursing experience (home health or hospice preferred).
- Certification: Current BLS certification.
- Skills: Strong critical thinking, time-management, and basic computer skills for electronic medical records.
