Minimum of 2 years CURRENT Home Health experience in Case Management and Hospice Experience
RNs will be providing Home Health & Hospice Case Management services to patients of all ages (infant to elderly)
Netsmart/Allscripts EMR experience strongly preferred
Mileage paid at current approved Federal IRS Rate (Mileage paid from office each morning)
