TravelNurseSource is working with Cynet Health to find a qualified Home Health RN in Manhattan, New York, 10038 Pay Information $2,351 per week About The Position Job Title: Registered Nurse (RN) - Home Health Job Specialty: Home Health Job Duration: 260 Days Shift: Day - 5x8 - 9am - 5pm, 40 Hours per Week Guaranteed Hours: Not Specified Experience: A minimum of two years of experience in a home setting within the last ten years License: Current RN Licensure in New York State Certifications: BLS required before the start date Must-Have: - Recent experience conducting comprehensive assessments of patients - Experience within a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care - Conducting assessments similar to UAS such as OASIS - Providing primary care to homebound persons - Experience managing care for individuals receiving home and community-based services, including Hospice and Assisted Living Job Description: - Perform initial in-home pre-evaluation of patients, assessing health status, strengths, care needs, and preferences, guiding individualized long-term care service plans. - Review consumer medical documentation and health referral forms relevant to the case. - Input evaluation data into an electronic form and transmit as required. - Document concerns, conflicting information, and other issues that arise during the evaluation process. - Emphasize continuity of care to reduce or eliminate fragmentation, duplication, and gaps in the treatment plan. - Discuss with consumers health care options, support needs, service vendor options, and waiver options. - Facilitate communication between the Call Center and eligible consumers for enrollment into a managed care health plan. - Enhance communication and collaboration with interdisciplinary care team members to improve care coordination and service delivery. - Collect quality review data and required documentation to support outcome measurements and record case notes into CRM. - Identify opportunities for health promotion and illness prevention. - Maintain a comprehensive working knowledge of community resources, payor requirements, and network services for the target population. - Meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template. - Perform other duties as assigned by the Regional Nurse Manager or other project management.
