Title: Home to Stay (HTS) Program Supervisor

Reports to: Assistant Director of Housing and Care Coordination

Job Objectives:

Responsible for the daily operation and supervision of the Home to Stay Program.

Provide Trauma Informed Care to all program participants, while utilizing a strength-based approach and person centered model.

Perform duties while actively promoting an environment consistent with MHA's Cultural Equity Policy and WELCOME Orange.

Responsibilities:

Responsible for completing required data collection, statistical, and program reports.

Monitor client payee accounts.

Complete monthly rent, rep payee, and check charts for HTS program.

Maintain HTS resident files according to Housing and Urban Development (HUD) / OMH compliance guidelines.

Maintain HTS resident data entry in HMIS system.

Assist with the HUD Annual Program Report and Annual OMH Case Management Program Report.

Responsible for program oversight and direct supervision of all HTS program staff and holding them accountable for program needs.

Ensure physical premises of HTS program site are maintained in a safe, clean and homelike manner.

Coordinate and/or provide 24 hour on-call coverage.

Maintain valid American Red Cross/American Heart Association CPR/First Aid Certification.

Facilitate development of peer support networks, program advisory councils and staff training.

Act as liaison to National Alliance on Mental Illness (NAMI) advisory committee.

Conduct HTS fire drills.

Monitor medication disbursement procedures for HTS Program.

Assist with the annual HUD renewals for HTS.

Establish and maintain linkages with community agencies and service providers.

Represent MHA at meetings such as SPOA and Coordinated Entry

Answer Home to Stay main line phone

Own / have a vehicle to drive to work and for program needs as needed (mileage reimbursement offered)

Other Duties:

Participate in MHA's committees as needed.

Answer agency phones as needed.

May be required to lift up to 25 pounds, if needed.

Requirements:

Qualifications:

Bachelor's Degree in related field with 2 years of experience working with individuals that are mentally ill or are seriously emotionally disturbed

OR

Associate's Degree with 5 years of experience working with individuals that are mentally ill or are seriously emotionally disturbed.

Supervisory experience required.

Effective written and oral communication skills.

Must be organized, self-motivated and have the ability to coordinate multiple tasks simultaneously.

Ability to exercise sound judgment under crisis situations and to abide by regulations regarding confidentiality.

Excellent interpersonal skills.

Excellent tact and diplomacy skills.

Excellent written and verbal communication skills.

Excellent attention to detail.

Excellent analytical, planning and organizational skills.

Valid NYS Driver's License and ability to drive with current driving record acceptable for agency insurance coverage.

Valid copies of personal auto-insurance, car inspection and registration required.

Terms of employment/benefits:

Exempt

Forty (40) hours per week with flexibility to include nights and weekends.
