Date Published: 03/24/2024REPORTING RELATIONSHIPSReports to: Nurse Manager of Hospice/Home Care Services or Assistant Nurse ManagerGENERAL SUMMARYWorks under the supervision of the Nurse Manager or Asst Nurse Manager. The RN Admission Nurse evaluates and admits patients for all home care disciplines and makes regularly scheduled visits as needed. Duties include: admission; revisit; OASIS data collection; developing the plan of care; referral of ancillary services e.g. rehabilitation therapists, social worker and home health aide and completion of required documentation. Assists with QAPI programs. Performs other duties as assigned.QUALIFICATIONSEducation:High School Diploma or successful completion of an equivalent High School Exam RequiredGraduate from an accredited school of nursing requiredBSN preferredLicensure:Licensed to practice as an RN in Ohio requiredLicensed to practice as an RN in Kentucky to be obtained after hire requiredBLS within 90 days of hire requiredExperience:Prior home care experience preferredInterpersonal Skills:Requires good communication skills. Has constant contact with patients, customers, physicians, supervisors and other members of the health care team.Essential Technical/Motor Skills:Must be able to stand and walk one to four hours a day.Sits four to seven hours a day. Drives one to three hours a day.Able to use hands for repetitive simple grasping and fine manipulation.Must have the ability to use feet for repetitive movement when driving.Bending, squatting, reaching, crawling and climbing are frequently required.Must maintain a current Ohio or Kentucky driver's license, provide own transportation and maintain current auto liability insurance.Must be knowledgeable and skilled in the use of all patient care equipment.Essential Physical Requirements:The physical demand level for this position is medium.Occasionally carries fifty pounds and walks at an intermittent self-pace.Essential Mental Requirements:Works under close supervision only occasionally and most of the time without the assistance of other personnel.Requires the ability to cope with frequent deadlines, adjust to irregular schedules and continuous concentration to details.High level analytical skills and ability to pay keen attention to detail required.Essential Sensory Requirements:Good vision and the ability to hear and speak clearly are required.Exposure to Hazards:Exposed to a variety of home situations and environments from adequate and clean to substandard and rodent or insect infested.Frequent exposure to adverse working conditions such as presence of communicable and infectious diseases.Frequent exposure to hazards of driving such as poor road conditions, ice and snow, and isolated roads.Other:The RN Admission Nurse is required to maintain a valid driver's license, reliable transportation and current auto liability insurance.JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONSThe following is a summary of the major job duties of this job. Other duties may be performed, both major and minor, which are not mentioned below. Specific activities may change from time to time.1. Performs admission visits. Initiates and completes the Physician's Plan of Care in collaboration with the patient, family, physician and other health care team members. Obtains informed consent for treatment. Informs patients both verbally and in writing of their rights and responsibilities as patients receiving Home Care and reviews the patient grievance procedure. Informs the patient, family or responsible party of available insurance coverage for Home Care services and non-reimbursable services. Provides to the patient/family information regarding the patient's rights to formulate Advance Directives. Instructs the patient/family in processes to follow if assistance is needed after regular office hours. Reviews general emergency preparedness instructions with the patient/family.2. Uses assessment skills and interviewing techniques to identify patient problems and patient care needs including teaching needs. Completes OASIS assessment. Collaborates with Coder and Intake Staff for correct ICD coding3. Formulates and initiates the plan of care including Home Care discharge planning, in collaboration with the patient, family, and/or caregivers.4. Makes home visits to render professional nursing care. Observes return demonstrations of health restoration/maintenance procedures.5. Completes clinical records for required documentation in accordance with Home Care standards. Maintains confidentiality of patient information.6. Serves as liaison between Home Care and physicians, SOMC hospitals, other health care team members and community agencies.7. Communicates changes in patient status to patients, family members, or caregivers, physicians and other health care team members.8. Participates in the 24-hour, 7 day a week on-call nursing coverage for patients/families as appropriate.Thank you for your interest in Southern Ohio Medical Center.
